Systems, methods, and devices for sacroiliac joint fusion described include an insertion instrument comprising a distal end, proximal end, and a ratcheting subassembly. The insertion instrument is configured to insert an implant across the sacroiliac joint, and the implant is configured to compress the sacroiliac joint. Compressing the sacroiliac joint can reduce motion thereof to promote fusion and stabilization.

Systems, methods, and devices for bony fusion are disclosed. A threaded cannulated implant may be inserted into a target space, such as in the sacroiliac joint. The implant may have a distal section, a proximal section, and a central section between the distal section and the proximal section. The distal section may comprise cutting threads and flutes for self-drilling the implant into bone. The proximal section may be configured to couple to an insertion instrument for the implant. The central section may have a lattice structure through which bone graft may flow to promote bone ingrowth. A solid core region may be located inwards from the lattice structure. The central section may also have openings through the core region to connect the lattice structure to the cannula.

An insertion device for deploying an implant includes an elongated main body having a distal locking portion for coupling to the implant and a proximal handle portion. The main body defines a central passage and the distal locking portion has outer ridges and slots to allow the outer ridges to flex radially inward when mounting to the implant. A plunger slides in the central passage for movement between an unlocked position for mounting the implant on the distal locking portion, a locked position for locking the implant on the distal locking portion, and an insertion instrument deployed position for deploying the actuation plunger to move the blades from the stowed position to the deployed position. A spike cap drive rotatably mounts on the main body having a socket end for engaging a drive nut on the implant to, in turn, move the spike cap.

A system and method for excising tissue from a stenotic spinal region is disclosed. The system may comprise a surgical kit including an access instrument, a bone reamer, a bone removal instrument, and a soft tissue removal instrument. The access device may be cannulated and utilized to achieve a path for the insertion and advancement of the other surgical instruments towards a target area, such as the interlaminar region. The bone removal and/or soft tissue removal instrument may be used at various angular orientations and depths within the lumbar region to ergonomically excise compressive features including hypertrophied ligamentum flavum. Minimally invasive and open procedures are disclosed.

An insertion instrument for inserting an implant includes an elongated main body having a proximal handle and a distal portion that selectively couples to the implant. A plunger is slidably engaged in a central passage of the elongated main body configured to fix the implant to the elongated main body by selectively filling the central passage within the distal portion. A hex nut driver is concentrically located about the plunger and elongated main body to deploy an actuation plunger of the implant. The proximal handle portion of the main body includes a staggered path therethrough for accepting a tab of the plunger therein. Advancement and retraction of the plunger tab within the staggered path alternates the insertion instrument between an unlocked position configured to mount the implant on the distal portion of the elongated main body, a locked position configured to lock the implant on the distal portion, and a deployed position configured to secure the implant.

A method, kits, and devices for performing the method of immediately improving and/or restoring the mechanical integrity and material properties of thermally shrunk collagenous tissue through delivery of a non-toxic protein crosslinker to the tissue. A thermal energy device shrinks the target tissue using controlled application of thermal energy. A non-toxic protein crosslinker is injected and/or topically applied to the target tissue before, during, and/or after thermal shrinkage. A dual applicator may comprise a thermal energy probe and injection needle. The thermal energy probe may apply thermal energy to the target tissue and the injection needle may apply the non-toxic protein crosslinker to the target tissue. A kit for use in performing the method may provide various components used for applying thermal energy and applying the non-toxic protein crosslinker to the target tissue.

Systems, methods, and devices for sacroiliac joint fusion described include an implant comprising a first body having a distal anchor and a second body having proximal anchor. The first body includes a threaded body integrally formed with the first body or threadably coupled to the first body. The distal anchor may be formed by a pair of deployable wings. The pair of deployable wings may be deployed within the cancellous bone of the sacrum and anchored against the cortical bone of the sacrum. The proximal anchor may engage with an outer surface of the ilium. The second body may be threaded along the threaded body to provide compression across the sacroiliac joint. Compressing the sacroiliac joint can reduce motion thereof to promote fusion and stabilization.

Systems, methods, and devices for sacroiliac (SI) joint fusion are described. At least one posterior SI implant and at least one lateral SI implant may be implanted. The posterior SI implant may be placed in the intra-articular region, and the lateral SI implant may transfix the SI joint such that the lateral SI implant passes through the ilium, across the SI joint, and into the sacrum. The lateral SI implant may compress the SI joint. The lateral SI implant may be inserted through a window in the posterior SI implant to interlock the two implants.

A disposable insertion instrument for inserting an implant includes a handle assembly, a drive assembly, and a plunger assembly. The drive assembly comprises an inner shaft and a spring received within an outer sleeve. A locking pin and drive lobes on the inner shaft cooperate to reversibly lock the drive assembly in a retracted position. A medical implant may be attached to the instrument via an adapter located at a distal end of the drive assembly. Following use for insertion of the medical implant into a patient, the disposable insertion instrument may be discarded.
