In some embodiments, a method includes delivering to a native valve annulus (e.g., a native mitral valve annulus) of a heart a prosthetic heart valve having a body expandable from a collapsed, delivery configuration to an expanded, deployed configuration. The method can further include, after the delivering, causing the prosthetic heart valve to move from the delivery configuration to the deployed configuration. With the prosthetic heart valve in its deployed configuration, an anchor can be delivered and secured to at least one of a fibrous trigone of the heart or an anterior native leaflet of the native valve. With the prosthetic heart valve disposed in the native valve annulus and in its deployed configuration, an anchoring tether can extending from the anchor can be secured to a wall of the heart to urge the anterior native leaflet towards the body of the prosthetic heart valve.

Apparatus and methods are described herein for anchoring a prosthetic heart valve. In some embodiments, an apparatus includes a tether attachment member that includes a base member that defines at least a portion of a tether passageway through which a portion of a tether extending from a prosthetic heart valve can be received therethrough. The base member defines a locking pin channel that intersects the tether passageway. A locking pin is disposable within the locking pin channel and movable between a first position in which the locking pin is at a spaced distance from the tether passageway, and a second position in which the locking pin intersects the tether passageway and can engage the portion of a tether disposed therein to secure the tether to the tether attachment member.

According to one aspect of the disclosure, a prosthetic atrioventricular valve system includes a prosthetic atrioventricular valve having a self-expandable frame with a collapsed and expanded condition. In the expanded condition, the frame includes (i) a body portion sized to be positioned within, and in contact with, a native atrioventricular valve annulus, (ii) an atrial flare portion positioned in an inflow direction relative to the body portion, the atrial flare portion having an outer diameter that is greater than an outer diameter for the body portion, (iii) a plurality of commissure attachment features (“CAFs”) extending in an outflow direction from the body portion, and (iv) a commissure support ring coupled to and circumferentially supporting the plurality of CAFs. A plurality of prosthetic leaflets may be mounted to the CAFs. The body portion, the atrial flare portion, and the plurality of CAFs may be formed integrally.

A prosthetic mitral valve with improved blood flow to the left ventricular outflow tract. The prosthetic mitral valve includes an expandable outer stent having an atrial end and a ventricular end, and an expandable inner stent attached to and at least partially positioned within the outer stent. The inner stent has an inflow end, an outflow end and a tether connector securing a tether. A valve assembly including a cuff and a plurality of leaflets may be disposed within the inner stent. The tether connector is positioned at the inflow end of the inner stent so as to shorten the overall length of the prosthetic valve.
